Two people have also been severely injured and immediately admitted to the nearby hospital. According to a few eye-witnesses, around 3 pm., the bike suddenly hit the shop of the street vendor. The vehicle came at a speed of about 100 KMPH.

The rider couldn’t be able to control the bike and smashed it against the shop. He was not wearing any helmet or other protective gear and due to severe injury in the head, he died on the spot. Nonapukur; 19th October 2021: Noapara highway again witnessed a devastating accident yesterday night. A Bus and a loaded truck collided face to face which caused the death of the bus driver. The name of the driver was Nutun Shaw(45). According to the eyewitnesses, the accident took place at around 11:30 pm.

Buses and Trucks often run quite recklessly on the highway and no police patrol comes forward to regulate rash driving, said a local street vendor. Moreover, the locals have also accused the municipality of not installing sufficient light sources around the area which causes very low visibility on the streets. Due to all these reasons, that particular area of Nonapukur has become an accident-prone zone.

According to several estimates, it is the 13th accident of the year, and in almost every accident, Nonapukur witnesses deaths. Fortunately, there were no passengers on the bus as well as no conductor as the bus was returning to the depot. Though the bus driver was killed, the truck driver got severely injured and was admitted to the district hospital. Baruipara; 15 June 2022: 28 people died in a devastating accident yesterday on an unregulated railway crossing. It was a bus that came on the railway track and collided with a train. The train was coming from Lalgola to Sealdah approximately at a speed of 110 KM per hour. The bus was completely smashed against the train which left no survivors. Among the passengers, there were 11 females, 13 males, and 4 children.

The train tried to break itself but most shockingly the train got derailed and resulted in severe injury to 11 passengers. The locals came in to help rescue the affected people and the fire brigade was informed. All of them were taken to the nearest rail hospital. Now questions are arising, how even in 2022 there are so many unregulated railway crossings all over the country.

THREE KILLED ON NH18 IN A FATAL MOTORBIKE ACCIDENT

Special CorrespondentPaikpara; 21 July 2022: Reckless driving again snatched the lives of three youngsters yesterday. It was yesterday night when a motorbike carrying three riders collided with a tree on NH18 and all fell to death. The riders were Female (26), Male (24), and Male (23) respectively. According to some local eyewitnesses, it was around 10:30 when the bike was coming from Gourdanga’s side at a fearsome speed of around 150 km per hour.

As soon as the accident took place, locals came to immediate help the riders. They took the three to the nearby hospital but unfortunately all were declared dead upon arrival. As per the report, none of the three riders was wearing helmets or taking any other safety precautions. Highway police patrol immediately started investigating the issue and trying to find out more details about the riders.

According to the primary investigations, all three were intoxicated and that’s why they lost control of the bike and hit the tree. The commissioner of police has again appealed to all the citizens to maintain proper safety norms while riding bikes and follow the ethics of Safe Drive, Save Life.

It was the day of 12th December, when I left my house for the office on my bike at 9 am, then as soon as I went some distance, I saw a live accident happening with my own eyes. The accident happened between a bus and a car. The accident scene is like that a car coming from the opposite side collided head-on with the bus coming from the right side.

The collision was so strong that the front portion of the car was completely crushed and due to this the two people sitting in the car died instantly on the spot. The car was a Suzuki company model Nexa series car with car number AB01C2345 and the bus was a local city bus operated by a local transporter with bus number CD06E7890.

There were a total of 26 people on the bus, in which two people were sitting in the front along with the driver and the rest were sitting in the seats of the bus. The car directly hit the left side of the bus from the front, due to which two people sitting in front also died on the spot and the driver was seriously injured. And all the other people who were on the bus also got serious injuries.

Immediately after the accident, local people started doing rescue, I also helped in the rescue. The police reached there within 10 minutes of the accident and the ambulance also reached within 15 minutes. With everyone’s help, all the injured were put in the ambulance and sent to the hospital.

After the rescue, the police sealed the accident car and bus and got the road cleared so that the vehicles passing through there did not face any problems. It was around 8 o’clock in the morning on Monday, November 6, when I left my house to go to school. After walking about 500 meters, I saw a terrible accident happening on the main road in which a bike and a car were involved. It was a sunny day on 5th November when I came to know from a neighbour that a major accident had happened in our locality. When I reached the spot, I saw that two cars had collided and both the cars were badly damaged. Immediately after the incident, police and ambulance reached there and started the rescue operation.

The accident was so bad that the victims had to get out by cutting off many parts of the car. An accident took place in our locality at around 10 am yesterday in which a car collided with an electric pole. Yesterday on 10th January at 1 pm I saw a horrific accident happening on the over bridge connecting A to B of our city. The accident was of a mini truck which collided with the divider of the overbridge. The number of the mini truck is CD01AB2345. The front part of the truck was badly damaged, due to which the driver got trapped inside the truck, and was taken out by cutting the door of the truck with the help of a cutter.

There was no loss of life in this accident but the truck driver suffered serious injuries. Soon the ambulance or police reached the site and the truck driver was taken out of the truck and then transferred to an ambulance and then sent to the hospital.

It was the time of 1st January when I was going home from my office at 4 o’clock when a massive accident happened 100 meters ahead of my car. The accident happened in front of the ABC showroom on the National Highway, in which a truck was moving about 200 meters ahead of my car and an SUV came from behind at high speed and collided with the truck.

The collision was so strong that the truck overturned after the impact and the SUV was completely shattered. All the dead and injured were put into ambulances and taken to the nearest hospital. With the help of cranes, the police removed the crashed vehicles from the road and dispersed the local crowd from the accident site. Police traced the contacts of all the victims and informed their families. Later, when the investigation report came, it was found that all the people sitting in the SUV were drunk, due to which they were driving the car very fast.

It was 2 o’clock on Friday afternoon, when I came out of my coaching centre, I saw that there had been an accident on the road in front of my coaching centre. Yesterday, 22nd June, at 4 pm, when I was coming home from school, on the way I saw live a massive collision between a car, a bus and a bike. All three vehicles came at high speed from different directions and collided at the turning point of the road. Step 1 Get identification and insurance information from the other driver.

Champaran; 21st November: In a world of advancing technology in the automobile sector where A.I. driving and active assist technologies are coming into the mainstream, the huge tally of deaths is still something that haunts both the government and the public. Yet again a serious accident took place yesterday at 3 P.M, on the city bridge, Champaran where there was a severe collision between a motorcycle and a car. The car was coming from the opposite side at a high speed and the motorcycle driver had no time to react. Unfortunately, the man driving the motorcycle, named Harish Tiwari (23) died on the spot and the woman, Deepika Tiwari (22) who was with him was lying on the road in an unconscious state. Both of them received severe head injuries due to the accident.

There were three people in the car and two of them were intoxicated. The driver of the car, Ranjan Chatterjee (32) was also almost dead due to a terrible collision with the motorcycle and the other one was lying unconscious with blood. Only the third guy sitting in the back seat of the car was in a conscious state, but he also broke his jaw and wrist due to the accident. Later he informed them that they were going to Siwan and the driver was heavily drunk along with his friend sitting in the front seat of the car.

Within no time, a large number of people gathered around the site of the accident. Most of them were busy recording the incident with their mobile phones, but some responsible people called the ambulance, and all the injured people were sent to the district hospital immediately without any delay. The damaged vehicles were also towed away from the road to clear the jam on the road.

After that, the police arrived on the spot to inquire about the accident. The police officers inspected the site of the accident and questioned the people about the accident. They also cleared the huge gathering of people which was causing the traffic jam on the road. Media personnel and reporters also arrived on the scene to report the incident. The police contacted the families of the victims of the accident. And both the vehicles were taken to the police station and a case was filed against the car driver due to his irresponsible behavior.

Subject – Report writing on a road accident

Date/Month/Year Thakurganj, Bilaspur.

On 12th March, when I was going to my sister’s house on the occasion of the Rakshabandhan festival, a truck, and a bus collided head-on on the way. The main reason for the collision was the speed of both the vehicles.

The bus was full of passengers and the truck was full of heavy cargo. As soon as the collision took place, the bus overturned due to a strong collision. Out of the total passengers on the bus, six died on the spot, all of whom were elderly.

Other passengers on the bus suffered serious injuries. Some had internal injuries and some had broken heads. The passengers who got more injuries were taken to a nearby hospital by the people there.

Out of the six people who died, two badly got stuck under the bus, and one’s hand was cut off. On the other hand, there was a fire in the truck due to which the goods loaded on that started burning. All the people were trying to extinguish the fire in the truck but the flames were getting stronger. There was no trace of the truck driver.

Suddenly the truck exploded, taking everyone away from both the vehicles. The people called the police on the spot. When the police officers reached there, they asked people about the incident. People explained everything in detail. An officer called JCB to get the bus straight and took both the vehicles into custody.

Along with this, the police identified all the dead people and informed their family members, and asked the concerned people to appear at the police station soon.

2. Write a Report on Road Accident in 200 Words | Sample 2

Subject – A report on a road accident

DD/MM/YYYY Meera Road, Jamshednagar.

Yesterday when I was returning home from the office, a very serious accident happened on Babulal Pulia. There was a collision between a motorcycle and a car. The man on the motorcycle died on the spot and the woman who was with him was lying on the road in an unconscious state.

There were four people in the car and all four were intoxicated. The driver of the car was also almost dead due to a horrific collision with the motorcycle and the other three were lying unconscious with blood.

Within no time, there was a crowd of people. Some responsible people called an ambulance, and all the injured people were sent to the district hospital.

After that, the police reached the spot. The police officers took stock of the incident site and questioned the people about the accident. Also, they cleared the crowd of people gathered there from the road.

The police called the relatives of both the vehicles to the police station and started an investigation into the incident.

3. Write a Report on Road Accident in 150 Words| Sample 3

5th August NH 27, Lucknow.

Last night, while I was going to meet my uncle, a very dangerous accident happened on NH 27. Actually, an uncontrollable dumper truck hit two young boys riding a scooter from behind. One boy died on the spot and the other continued to suffer on the highway till the ambulance reached the spot.

Within a few minutes, the highway got jammed. The dumper driver had run away. Some people called the ambulance and some called the police. When the ambulance arrived, it took both the seriously injured boy and the dead boy to the hospital.

After almost five minutes, the police reached the spot and got the jam reduced. The phone of one of those boys was found on the spot and with the help of the phone his family was informed.

The police took the scooter and the dumper to the police station with the help of some of their co-workers and also informed about the accident to the dumper owner with the help of the vehicle number for further investigation.

4. Write a Report on Road Accident in 100 Words | Sample 4

Subject – A report on a road accident in my locality

9th July, Wednesday Sri Nagar Locality.

A terrible accident happened on the main road of Srinagar locality in which a milk vehicle trampled an elderly person. The old man died on the spot. The actual reason for the accident was the drowsiness of the driver who couldn’t sleep last night due to some reasons.

When this incident happened, the people of the locality ran and caught the driver. Some people even beat him up.

Some sensible people in the locality immediately informed the police and after some time the police reached the spot. The people handed over the driver to the police. The police sent the driver to the police station and his vehicle was confiscated so that the police could start further proceedings.

The chairman of the locality expressed grief over this and talked about paying special attention to the elderly.

5. Report Writing on Road Accident for Class 12 | Sample 5

DD/MM/YYYY Agra Bypass, Bangarmau.

A heart-wrenching accident happened on the Agra Bypass in Bangaramu. The accident happened when the traffic light on the bypass was red.

Everyone was waiting for the green signal when a speeding truck driver from behind hit the cars and bikes in such a way that four to five cars and some motorcyclists got hit by it.

Around 10 to 12 people perished immediately and others were badly injured. Within seconds people gathered. When the local people caught the truck driver, he told that the break of his truck had failed. The local people caught him and took him to a safe place to save him from the mob and immediately called the police.

Police reached the spot, took the driver into custody, and got the damaged vehicles to one side of the road to clear the hours-long jam.

The police sent the dead people to the hospital for post-mortem and their families were informed. Along with this, the injured people were sent to the hospital for treatment with the help of three ambulances, and further investigation was started by the police.

Incident Report Samples to Help You Describe Accidents

Your company’s incident report form should make it easy to record the details of a workplace accident. Even fill-in-the-blank forms require a long-form description of the incident. This portion of the report can be the most difficult part to write.

In this article, we provide completed incident report samples to help you hone your documentation skills.

What Does an Incident Report Need to Include?

An incident report is a formal recording of the facts related to a workplace accident, injury, or near miss . Its primary purpose is to uncover the circumstances and conditions that led to the event in order to prevent future incidents.

Every incident report you file should contain a minimum of the following:

Type of incident (injury, near miss, property damage, or theft)
Date of incident
Time of incident
Name of affected individual
A narrative description of the incident, including the sequence of events and results of the incident
Injuries, if any
Treatments required, if any
Witness name(s)
Witness statements
Other workers involved
Video and/or 360-degree photographs of the scene

Include quantifiable measurements where possible. For example, the ladder capacity is 250 lbs and the victim was hoisting 300 lbs.

Finally, where more than one person is injured in an incident, create a unique report for each affected employee . While it’s fine to duplicate general details between reports of this nature, you’ll need to include details specific to that person, such as the event from their point of view or medical records.

When Does an Incident Report Need to Be Completed?

Create an incident report as soon as your employees are safe , which includes seeking medical attention and implementing an immediate corrective action to prevent further danger or damage.

By recording details immediately, you improve the accuracy of your report and the effects of your corrective actions. While it may take a few days to complete your report, it should take you only hours (or less) to start it.

Depending on your company policies and oversight from relevant regulatory bodies, you may be legally bound to complete an incident report within a given amount of time. If the incident resulted in a recordable injury, you must complete OSHA Form 301 within seven days. In addition, you may need to send your report to the organization providing oversight, US Federal or State OSHA , for example, within a given period of time.

Review federal OSHA incident reporting and recordkeeping requirements or visit your state OSHA website for details. Remember that you must also report a fatality, hospitalization, or amputation directly to OSHA.

Incident Reporting Mistakes to Avoid

Incident reports are more than a mandatory form to complete: they’re a vital part of your recordkeeping practices both for compliance and safety in general. Their importance means you want to get as much relevant information as you can as quickly as possible. But sometimes, safety teams can get in their own way and miss out on important details by making common mistakes.

Blame-casting and just getting it done are two common incident reporting mistakes you should avoid.

It is easy to go on a witch hunt when accidents happen. However, incident reports aren’t created for the purpose of finding out who’s to blame. While staff should be held responsible (as stipulated by company policy) when they knowingly endanger themselves others, an incident report should focus on improving workplace safety.

If you make it clear you’ll abide by company policy but are not out to cast blame, you increase the likelihood that employees will participate in your incident investigation and provide honest witness statements. These factors create a stronger incident report.

In addition, if you rush through the report to “just get it done,” you may miss out on important details or lack clarity in wording. Irresponsible reporting can lead your team into even more danger, while detailed, clear reporting can lead to improved work conditions and better training.

Three Incident Report Samples

Below are some sample incident report formats for three common types of workplace accidents. Use them as guides to effectively describe events.

Injury and Lost Time Incident Report Sample

If an injury requiring medical treatment, lost time/altered responsibilities happens in your workplace, it’s important to document it ASAP while the details are still fresh in memory.

Here are some of the vital elements to include in your description of the incident:

Location (Address)
Date/time of incident
Name of supervisor
Description of the incident, including specific job site location, the sequence of events, and the results of the event
Whether or not proper PPE was being used
The root cause(s) of the incident
Associated hazards raised and resolved following the event
The affected individual’s version of the events
Actions taken by concerned individuals after the incident
Description of injuries
How the decision was made to call (or not to call) emergency services
Treatment required
Photographs of the scene

Though the details above seem excessive, mentioning them in the incident report paints a more accurate picture. It’s important to include the above information in as detailed and concise a manner as possible. Holes in your report could lead to inferences and missed opportunities to create a safer workplace.

To write an incident report, use a narrative format. In other words, simply tell the story. Here’s an incident report sample description of a slip or fall resulting in a fracture, written in narrative form:

“On Friday afternoon, February 3, 2019, at 2 p.m. in ABC Shipping Co. located in 13th Avenue, Applewood, one of the warehouse workers (John Keegan) slipped and fell while carrying heavy (85 lbs) inventory. The root cause is believed to be the unavailability of a hand truck or pallet jack. Instead of waiting, John attempted carrying the load himself. A second possible cause is the condition of John’s PPE, specifically his boots, which are very worn in certain places.

When John fell, his colleagues rushed to his assistance. Suspecting a fracture, the supervisor on the floor (Kathy Pickens) decided to call 911. John said he felt a bit dizzy when he lost his balance and that he just ‘tripped over his feet.’ He tried to minimize damage to the load itself while falling, which may have led to a more awkward fall.

Two of his co-workers said that they noticed he was struggling a bit before the fall, but were busy with their own tasks and felt it would be rude to ‘call him out.’ See the attached witness statements for more info.

John was taken to a nearby hospital and a fracture to his wrist was confirmed. John will be out of work for a number of weeks.

The supervisor is currently working with the safety officer (Chris Darnell) to assess the condition of the floor, the number and condition of hand trucks and pallet jacks on the floor, as well as the condition of company-provided boots over two years old.”

As you can see, including the full story, complete with small details and witness statements will help you investigate and recall the incident with greater clarity.

After documenting the incident, you may have legal reporting requirements. Report and store the files according to recordkeeping requirements from regulatory bodies. It’s generally best practice to preserve the files for the duration of employment.

Exposure Incident Report Example

When exposure to dangerous chemicals or pathogens occurs, it’s important to document the event carefully in an incident report.

Including the following details will make your exposure incident report more effective:

Name of affected individual
Name of Supervisor
Description of the incident, including specific job site location, sequence of events, and results of the event
Name of the chemical/pathogen/carcinogen, amount, concentration, and description of hazard labels/safety data sheets (SDS)
Exposure monitoring data, if available
Whether or not a contaminated sharp was involved
Description of injuries, including body part(s) exposed, length of exposure, and size of area exposed
Signs and symptoms displayed
Photographs of the scene and hazard labels involved

Here’s an incident report sample description for overexposure via inhalation:

“Josh Lee, a freight handler in XYZ Shipping Lines, was exposed to carbon monoxide fumes on December 2, 2017, Tuesday, from (estimated) 7:30 AM to 11:30 AM. He was at the unloading bay B, helping unload some freight from various containers with the help of two forklift operators: Kit Stevens and Donald Summers, neither of which complained of symptoms.

During unloading, Lee suddenly experienced lightheadedness and nausea. He informed his supervisor (Donna Martin) that he thought he was ill.

Martin noticed his symptoms were consistent with CO exposure, so she walked over to the area and felt the air quality seemed off. She ran a sensor (Portable Direct Reading Monitor) and discovered that CO levels were on the high end but within the acceptable limit: 30 PPM.

Stevens and Summers were ordered to turn off powered vehicles and sit in fresh air for an hour while the ventilation system could be examined (see attached report).

Lee was driven to the hospital by Martin to receive treatment. On the way, he complained of blurred vision briefly but said that it had cleared up upon walking into the ER. A full report will be attached once received.

Lee’s colleagues, Stevens and Summers, didn’t notice anything out of the norm with the air quality, but Summers acknowledged that at 11:00 AM, his vehicle had been left running near the open end of a container for thirty minutes while Lee was adjusting two improperly arranged pallets just inside. There is no air quality data from that time.”

Be sure to attach medical reports and lost-time from work, if any, to your initial report. Keep the report on hand for the duration of the employee’s tenure with the company, at a minimum.

First Aid Incident Report Sample

Some workplace mishaps result in an injury that requires on-site or walk-in medical first aid treatment. Often, workers can return and finish their shift. Recording an incident report is still vitally important, even where it’s not expressly required by regulatory bodies.

Here is an incident report sample description for incidents resulting in the need for first aid.

“ On May 12, 2019, at around 9:34 AM. at King Street job site, Michael Williams was hit by an air nail gun that had been dropped by Carl Simone near the top of a staircase and gained momentum as it tumbled down.

Williams, who was nailing drywall at the bottom of the staircase and wearing noise protective headphones, eye protection, and a short-sleeved shirt, was hit in the arm, causing a bruise and abrasion. He was treated with antiseptic, antibiotic ointment, a bandage, and an ice pack on site. Williams returned to work within a half hour.

Simone had shouted a warning after he dropped the gun, but Williams said he did not hear it. Simone said that he simply lost his grip, but Williams said he felt that the tool may have been ‘swung’ before dropping. There were no other witnesses to report on the circumstances of the accident.

Simone was wearing gloves when he dropped the nail gun. There are no outstanding hazards related to this incident and all workers will be reminded about glove selection and tool handling at the next safety meeting.”

While you should always document injuries requiring first aid internally, you typically don’t need to add them to your Form 300A or other regulatory reporting. In fact, doing so can needlessly elevate your incident rate.

SA road crash between truck and ute claims lives of woman and man north of Roseworthy

The scene of a crash between a ute and a truck.

A woman and a man have been killed in a horrific crash involving a ute and a truck on a South Australian regional road.

The victims, both aged in their 60s, were in the Toyota ute when the vehicles collided on Owen Road at Magdala in the state's lower north just after 2pm on Friday.

The woman and the man were from the local area, police confirmed on Saturday morning.

"Sadly, the two occupants of the ute, a 67-year-old man and 63-year-old woman … died at the scene," police said.

A power pole and power lines were knocked down in the crash.

SA Police's Major Crash team is investigating the incident and police have appealed to anyone with information to contact Crime Stoppers.

Magdala is a small locality north of Roseworthy, and is not far from the townships of Templers, Wasleys, Hamley Bridge and Freeling.

Images of the aftermath reveal a horrific scene, with the upturned ute appearing to have been caught under the cabin and chassis of the truck.

The truck's driver, a 65-year-old Smithfield man, was not injured, police said

Adelaide cyclist in critical condition

The Magdala crash happened a short time before an Adelaide cyclist was rushed to hospital in a critical condition after being hit by a car at Salisbury East.

Part of I-84 in Patterson closed after 7-vehicle crash with overturned dump truck

Part of I-84 in Patterson is closed after a multiple vehicle crash happened shortly after 11 a.m. Tuesday morning on the eastbound side of the highway.

New York State Police trooper Aaron Hicks said the crash involved seven vehicles, including an overturned dump truck near mile marker 60.8 eastbound, west of exit 61 and close to the exit for NY Route 311.

Hicks said multiple fire, police and EMS units responded to the scene for minor injuries.

All traffic is currently being diverted to Luddingtonville Road in Patterson.

1 killed in rollover crash on northbound I-15 near Valley Center

A deadly crash on I-15 in North County San Diego created a slow-down during Tuesday's morning commute.

A truck with two people inside crashed and rolled over on northbound I-15 near Deer Springs Road near Valley Center at about 6:35 a.m., according to California Highway Patrol. One person was thrown from the vehicle, which came to a rest on the right-hand shoulder.

One person died in the crash, according to CalFire capt. Mike Conrette. The other person was transported to the hospital with serious injuries.

One right lane was blocked as crews responded to the scene. Footage from SkyRanger 7 showed debris scattered around a white truck. At least two fire trucks were at the scene. A green freeway sign was down near the crash scene, but it was not clear if it was a result of the accident.

Murkomen Gives Update on New Measures to Curb Road Accidents

T ransport Cabinet Secretary Kipchumba Murkomen has urged Kenyans to adhere to traffic rules, attributing the rising number of crashes to recklessness.

Addressing worshipers on Sunday, at AIC Milimani in Nairobi, Murkomen said the government, through relevant agencies would roll out stringent measures to tame road carnage.

“Many of the road accidents happening in the country are a result of outright recklessness,” said Murkomen.

Despite the destruction of road infrastructure by floods, fear of road crashes could worsen, the CS insisted on installation of foot bridge in many parts of the city including Mathare, Mukuru and Kibera.

“We are installing foot bridges in many parts of the city including Mathare, Mukuru and Kibera,” he said.

However, the CS said that pedestrians especially those in big cities were to bear the biggest brunt of road deaths.

“Pedestrians, especially in the big cities, continue to bear the biggest brunt of road deaths and this must stop,” he remarked.

He also assured Kenyans that as a ministry, they are doing everything possible to ensure the roads are safe for Kenyans including installation of speed cameras, getting NTSA to crack down on unroadworthy vehicles and irresponsible motorists.

CS Kipchumba Murkomen On Road Accidents

CS Murkomen added that the ministry was looking forward to even privatize motor vehicle inspection.

“As a Ministry we are doing everything possible, including the privatization of motor vehicle inspection, installation of speed cameras and getting NTSA to crack down on unroadworthy vehicles and irresponsible motorists” Murkomen stated.

In addition, he explained that the ministry was going after rogue drivers to reduce road accidents. “We are reining in errant drivers,” he added.

In October 2023, NTSA unveiled the National Road Safety Action Plan (2023-2027) which aims to achieve a 50 per cent reduction in severe injuries and deaths.

He also called out citizens to be responsible and give the government easy time to restore roads and bridges countrywide.

“With collective responsibility, this can be controlled as the Government begins restoration of roads and bridges countrywide,” CS Murkomen concluded.

Report on road traffic deaths.

Road safety remains a critical issue globally, with the World Health Organization estimating over 1.35 million deaths annually due to road traffic accidents.

The report also stated that road traffic accident had fallen slightly and the efforts to improve road safety had a positive impact and that significant reductions in road traffic deaths could be made if proven measures are applied.

Furthermore, road traffic injuries remain the leading killer of children and young people aged 5-29 years with more than half of the fatalities occurring among pedestrians, cyclists and motorcyclists.

“More than half of fatalities occur among pedestrians, cyclists and motorcyclists, in particular those living in low and middle-income countries,” the report read.

It adds, “Urgent action is needed if the global goal of at least halving road traffic deaths and injuries by the year 2030 is to be achieved.”

Scottie Scheffler arrested for allegedly assaulting officer near fatal crash while on way to PGA Championship

Masters champion Scottie Scheffler was arrested by police in Louisville on Friday morning on his way to the PGA Championship, with stunning images showing him handcuffed as he was led to a police car and charged with assaulting an officer after allegedly attempting to drive around traffic backed up from a pedestrian fatality investigation. Scheffler was released later in the morning and continued competing in the tournament.

Police said Scheffler was booked for second-degree assault of a police officer, third-degree criminal mischief, reckless driving and disregarding traffic signals from an officer directing traffic. In a statement to CBS News, Scheffler's attorney Steve Romines said the golfer "never at any point assaulted any officer with his vehicle" and added that "we will litigate this matter as needed."

In a statement posted to Instagram , Scheffler said he was in a "very chaotic situation."

"There was a big misunderstanding of what I thought I was being asked to do," Scheffler said. "I never intended to disregard any of the instructions. I'm hopeful to put this to the side and focus on golf today."

Police said a pedestrian had been struck by a bus while crossing the road in a lane that was dedicated to tournament traffic. Police said the man died at the scene, CBS affiliate WLKY reported . Traffic was backed up for about a mile in both directions on the only road that leads to Valhalla Golf Club, with dozens of police cars flashing red-and-blue lights near the entrance.

Scheffler, the No. 1 player in the world, later started the second round shortly after 10 a.m. EDT. After finishing Friday's round, Scheffler told reporters he wouldn't discuss specific details about the incident, but he said he didn't know at the time that the accident ahead of him in traffic was fatal.

"I feel like my head's still spinning," Scheffler said. "I can't really explain what happened this morning. I did spend some time stretching in a jail cell — that was a first for me."

Scheffler expressed confidence that the charges would "get resolved, I think, fairly quickly."

Why was Scottie Scheffler arrested?

According to a police report, Scheffler was in an eastbound lane and pulled into the westbound lanes, where a uniformed detective was standing. The detective stopped Scheffler and attempted to give him instructions, but Scheffler "refused to comply and accelerated forward," according to the report.

The detective, identified as Det. Gillis, was dragged "to the ground" and suffered "pain, swelling, and abrasions to his left wrist," according to the report.

The detective was dressed in a high visibility reflective jacket when he stopped Scheffler's car to give instructions, the arrest sheet said. Gillis was taken to the hospital for his injuries.

The Louisville Metro Police Department said that all lanes were closed during the traffic fatality investigation.

"While an officer was directing traffic, an encounter with a motorist attempting to make entry into a restricted area ultimately led to the driver being arrested," the police said in a statement. "We are in the process of conducting a thorough investigation. We are appreciative that all parties involved are fully cooperating."

Romines said Scheffler was driving a marked player's vehicle and proceeding as he'd been instructed.

"I think the officer that was directing traffic was maybe not part of the event traffic detail and so that's where the miscommunication arose, and that's why we're here," Romines told WLKY.

ESPN reporter Jeff Darlington said he watched this unfold with Scheffler driving past a police officer a little after 6 a.m. in his SUV. He said police pulled Scheffler out of the vehicle, pushed him up against the car and immediately placed him in handcuffs.

"Scheffler was then walked over to the police car, placed in the back, in handcuffs, very stunned about what was happening, looked toward me as he was in those handcuffs and said, 'Please help me,'" Darlington said. "He very clearly did not know what was happening in the situation. It moved very quickly, very rapidly, very aggressively."

Darlington said whole the incident started with a "misunderstanding with traffic flow."

With cars backed up in the morning darkness, other PGA-marked vehicles tried to move slowly toward the entrance. Traffic finally began to move slowly a little before 7 a.m.

A spokesperson for the PGA of America identified the man who was struck by the shuttle bus as John Mills. In a statement, the PGA's Greg Dillard said Mills, a worker with one of the group's vendors, was going to work when he was hit.

"This is heartbreaking to all of us involved with the PGA Championship," the PGA said in a statement. "We extend our sincere condolences to their family and loved ones."

Kentucky Gov. Andy Beshear conveyed his condolences on social media , saying, "Our hearts are broken, and we continue to pray for the victim's family."

"The events that occurred afterward are the unfortunate result of this tragic incident, and we are hopeful that all parties involved can come to a resolution," Beshear said.

Scheffler also expressed his sympathies for the man who died.

"It truly puts everything in perspective," Scheffler said on social media.

The website for the Louisville Metropolitan Department of Corrections briefly showed a booking photo of Scheffler before the photo was removed. According to the website, Scheffler was released at 8:40 a.m.

Did Scottie Scheffler play today?

Scheffler returned to the course at 9:12 a.m. He made his way to the practice area around 9:30 a.m. and was welcomed by fans — one shouted "free Scottie!"

Scheffler seemed like his normal, relaxed self, sharing a few laughs on the driving range. Then he went out and made a birdie on his first hole of the day after sticking his approach shot to three feet. Scheffler finished Friday's round two strokes behind leader Collin Morikawa at nine strokes under par for the tournament, CBS Sports reports .

It was a surreal start to what already has been a wild week of weather — the Masters champion and top-ranked in the world, dressed in workout clothes with his hands in cuffs behind his back amid flashing flights.

Darlington said police were not sure who he was. He said an officer asked him to leave and when he identified himself being with the media, he was told, "There's nothing you can do. He's going to jail."

Darlington said another police officer later approached with a notepad and asked if he knew the name of the person they put in handcuffs.

Scheffler is coming off four victories in his last five tournaments. He was home in Dallas for the last three weeks waiting on the birth of his first child, a son who was born on May 8.

"I feel like my head is still spinning. I can't really explain what happened this morning," Scheffler said after remarkably posting another 5-under 66 Friday that kept him in the mix for a second straight major championship. He is seeking to become only the fifth player since 1960 to win the first two majors of the year.

Last month, Scheffler won his second Masters Tournament victory in three years, finishing 11 under and four strokes up on his closest competitors.
